Surfaced by /code-review low on the #254 diff (plugins/os-backlog/lib/backlog/issues.rb): (1) ForgejoBackend/GhBackend#comments rescue StandardError to [] — a transient tea/gh failure makes decision-sweep's idempotency guard fail open and re-post comments; (2) FileBackend#add_label returns true when the file has no labels: line, silently dropping the label.
