Child of #333. After #330+#332. Instrument hooks/post_tool_use.rb to log one real event, refresh cache, one real Agent call; confirm tool_name==Agent and tool_input.subagent_type present. Existing tests manufacture these fields — only a live event is evidence. Update dispatch-hook.md.

Done: Live-captured a real PostToolUse event: hook is armed and firing; tool_name Agent, tool_input.subagent_type, cwd confirmed; tool_use_id present (docs corrected); async dispatches fire the hook at launch not completion

Evidence: Capture from live session 2026-08-15; docs commit eacd3e5687; instrumentation reverted and cache refreshed
