Replace stub gates with real states one TDD slice at a time, in the documented order: red-assert, lint, reviewer, escalation (architecture.md §Rebuild sequencing). One commit per state. Split this ticket into per-state tickets once ticket 1's design settles the schema. Depends on #328, #329.

Superseded: #328 (design dependency) closed. Real states already grown via #341 waves + #342 adapters (verdict gate, autocorrect_step, TeaTracker, PushNotifier - see plugins/os-sdlc/lib/os_sdlc/runner/, git log e8329f0..18803bd). Phase-3 skeleton plan is moot.
