Audit session fdee577b (2026-07-13): when review/diagnosis agents converge on a confirmed defect list, implementing that list must be a fresh delegation decision, not a continuation of the main loop's review thread. Add wording to plugins/os-context/prompts/session-start/10-orchestration.md naming review-convergence as a delegation gate.
