#197 (harden os-sdlc implement pipeline — surviving scope after carve-out vs #195)

What to build

Two SKILL.md hardening items from #197: (1) a default timeout plus output-redirect-and-tail convention on every gate-invoking Bash call; (2) an explicit prerequisite statement that setup-project must complete and be committed before implement is invoked.

Acceptance criteria

Every gate-invoking Bash call in SKILL.md carries the timeout + redirect-and-tail convention
setup-project prerequisite stated at the top of the implement flow
Passes AiDD artifact linting for the changed SKILL.md

Implemented on branch worktree-spec-195-impl (commits 12d76a2, 9e3fece, f5a4961). Suite 498 runs / 0 failures; opus critique + corrections applied; aidd-lint clean of blocking offenses.
