#197 (harden os-sdlc implement pipeline — surviving scope after carve-out vs #195)

What to build

When green-assert exits 1, route the retry by the failing file's path: failures in test_path go to a fresh test-writer, all others to a fresh programmer. SKILL.md step 5 currently routes everything to programmer unconditionally. Phrasing must compose with the fresh-agent-every-round rule landing in #221 (no resumes; distilled briefs).

Acceptance criteria

Retry routing decision documented in SKILL.md and driven by failing-file path
test_path failures dispatch a fresh test-writer; others a fresh programmer
Composes with the #221 fresh-agent rule (no SendMessage resume wording)
